Deals with families exhibiting complex family dynamics. Communicates with interdisciplinary team regarding the discharge planning status of all patients referred by them. Educates patient/family and physician regarding post-acute options and addresses freedom of choice and financial concerns. Clinical assessment and intervention: Conducts psychosocial assessment and interventions; makes treatment recommendations and engages patients/families in adherence to the treatment plan by reducing or eliminating, to the extent possible, the social, psychological, financial, behavioral and regulatory barriers to successful medical outcomes. Serves as a resource person and provides counseling and intervention related to treatment decisions and end-of-life issues. Advocates for patient and family. Patient and Family Support in Legally Complex Cases: Provides intervention in cases involving child abuse/neglect, domestic violence, elderly abuse, institutional abuse and sexual assault. Advocates for patient and family empowerment and independence to make autonomous health care decisions and access needed serves within the health care system. Meets Core Social work competencies, including adhering to regulations. Stays current with regulations which impact the care of patients and the Clinic's regulatory compliance. Promotes individual professional growth and development and meets mandatory/continuing education, skills competency, supports department-based goals which contribute to the success of the organization; serves as a preceptor, mentor, and resource to less experienced staff. Represents the department on committees. Other duties as assigned.
Education: Master's Degree in Social Work (or Clinical Counseling within Behavioral Health)
Experience: Hospital Social Work experience preferred. Exposure and experience in pre-acute and post-acute care. Knowledge of community resources preferred. Excellent communication and negotiation skills necessary to work collaboratively with multidisciplinary teams for optimal outcomes. Demonstrated ability to work independently as well as with teams. Must be flexible, open-minded and adaptable to change.
Licensure/Certification/Registration: Licensed Social Worker (or Counselor within Behavioral Health) in the State of Ohio. Certification as a clinical social worker or case management certification within one year of eligibility. Eligible licensure includes Licensed Social Worker (LSW), Licensed Independent Social Worker (LISW), Licensed Independent Social Work Supervisor (LISW-S), Professional Counselor (PC), Professional Clinical Counselor (PCC), Supervising Professional Clinical Counselor (PCC-S) through Ohio Counselor and Social Worker Board. Summary: Reviews and evaluates RAW EEG and video data. Responsible for daily scanning, documentation and annotation of ictal, interictal and paroxysmal events. Maps interictal and ictal patterns and creation of permanent file clips which represent these findings. Details patient history from patient interviews and writes daily monitoring progress notes in the patient hospital charts. Edits and archives EEG and video data of patients Video/EEG monitoring.
Job Responsibilities: Performs long term monitoring of adult and pediatric patients using closed circuit and digital audio/visual equipment and electrophysiological recording instruments. Presents patient history and evolution, EEG and video data and radiologic studies in daily patient rounds for Staff Physicians. Describes current seizures and frequency. Lists of current medications and results of previous investigations. Describes examination made during admission to the monitoring units. Maps, classifies an impression of ictal and interictal EEG/Video data. Writes final report and detailed description of EEG patterns, seizure semiology, radiologic findings, mapping of ictal and interictal abnormalities and selection of representative EEG samples. Serves as an instructor for new personnel being trained to perform testing and care for long term monitoring patients. Other duties as assigned.
Education: As necessary to meet experience and licensure requirements.
Experience: Minimum two (2) years recent technical experience in epilepsy long term monitoring, routine EEG lab with exposure to long term monitoring on Bedside Monitoring systems or any combination of such experience.
Licensure/Certification/Registration: Registered EEG Technologist (R/EEG) with Cardio Pulmonary Resuscitation (CPR) Certification.
Physical Requirements: Manual dexterity in order to place electrodes precisely and to operate recording equipment. Ability to assist with positioning and transporting of patients, pushing beds, carts and wheelchairs. Must be able to lift and carry items weighing up to 50 pounds. Possible exposure to communicable diseases and body fluids. Requires corrected hearing and vision to normal range.

Basic Life Support (BLS) Certification in all areas or successful completion of Basic Life Support (BLS) in new hire period. For critical care areas need Advanced Cardiac Life Support (ACLS) Certification in Critical Care or must obtain same within first year of employment. For Emergency Service Trauma areas need Trauma Nursing Core Course (TNCC) or must obtain within first 18 months of employment
Physical Requirements: Requires full range of body motion including handling and lifting patient, manual and finger dexterity, and eye-hand coordination. Operates ancillary equipment adherent to specific procedures. Checks and prepares patient prior to Vascular-Interventional study. Screens patient for ability to undergo procedure by understanding baseline lab requirements. Understands primary purpose of drugs used in IR, indications, contraindications, contrast drug interactions and major effects. Positions patient to obtain radiographic exposure, adjust exposure factors and process images. Uses professional judgement to adapt imaging and therapeutic procedures to improve diagnostic quality and therapeutic outcome. Knowledgeable in acquiring, archiving, transmitting, and printing images via PACS - when applicable, knowledgeable in after hours transmission of images and appropriate paperwork. Immediately reports any malfunctions to help desk and PACS Administrator. Knowledgeable in billing via RIS. Records and maintains procedural data to include injection data, elapsed fluoroscopic time and procedural information. Provides after hours standby call in service. Monitors equipment, systems, cleans equipment and effects timely repair. Performs teaching and instructional service to students and new hires. Continually strives to lean new technical information and skills through attendance of periodic in-service programs and participation in professional affiliation sessions. Other duties as assigned.
Education: Graduate of an AMA approved school of Radiologic Technology.
Experience: Minimum one year experience as radiologic technologist preferred. Preference given to previous experience in high volume angiography department.
Licensure/Certification/Registration: Must be Registered Radiologic Technologist-(ARRT). State licensure is required. Prefer those registered in Vascular and Interventional Technology (CV or VI). Must have current Cardio Pulmonary Resuscitation (CPR) license, prefer Advanced Cardiac Life Support (ACLS). Physical Requirements: Requires standing and walking for extended periods of time.
