Summary: Coordinates the care and services of select patients across the continuum of illness. Job Responsibilities: Transition of Care planning: Manages all aspects of discharge planning for assigned patients in order to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers. Deals with families exhibiting complex family dynamics. Communicates with interdisciplinary team regarding the discharge planning status of all patients referred by them. Educates patient/family and physician regarding post-acute options and addresses freedom of choice and financial concerns. Clinical assessment and intervention: Conducts psychosocial assessment and interventions; makes treatment recommendations and engages patients/families in adherence to the treatment plan by reducing or eliminating, to the extent possible, the social, psychological, financial, behavioral and regulatory barriers to successful medical outcomes. Serves as a resource person and provides counseling and intervention related to treatment decisions and end-of-life issues. Advocates for patient and family. Patient and Family Support in Legally Complex Cases: Provides intervention in cases involving child abuse/neglect, domestic violence, elderly abuse, institutional abuse and sexual assault. Advocates for patient and family empowerment and independence to make autonomous health care decisions and access needed serves within the health care system. Meets Core Social work competencies, including adhering to regulations. Stays current with regulations which impact the care of patients and the Clinic's regulatory compliance. Promotes individual professional growth and development and meets mandatory/continuing education, skills competency, supports department-based goals which contribute to the success of the organization; serves as a preceptor, mentor, and resource to less experienced staff. Represents the department on committees. Other duties as assigned.

Education: Master's Degree in Social Work (or Clinical Counseling within Behavioral Health)

Experience: Hospital Social Work experience preferred. Exposure and experience in pre-acute and post-acute care. Knowledge of community resources preferred. Excellent communication and negotiation skills necessary to work collaboratively with multidisciplinary teams for optimal outcomes. Demonstrated ability to work independently as well as with teams. Must be flexible, open-minded and adaptable to change.

Licensure/Certification/Registration: Licensed Social Worker (or Counselor within Behavioral Health) in the State of Ohio. Certification as a clinical social worker or case management certification within one year of eligibility. Eligible licensure includes Licensed Social Worker (LSW), Licensed Independent Social Worker (LISW), Licensed Independent Social Work Supervisor (LISW-S), Professional Counselor (PC), Professional Clinical Counselor (PCC), Supervising Professional Clinical Counselor (PCC-S) through Ohio Counselor and Social Worker Board. Certification options include Certified Case Manager (CCM), Accredited Case Manager (ACM), Certified Social Work Case Manager (C-SWCM), Certified Advanced Social Work Case Manager (C-ASWCM), Certified Social Worker in Health Care (C-SWHC), or other specialty certification as appropriate to the service.

Physical Requirements: Requires direct patient contact. Requires extensive walking, telephone use, computer use, bending, reaching and lifting items weighing 20 pounds or less. Must be able to work well under pressure and maintain professional demeanor under potentially adverse conditions. Extensive reading of medical terminology, writing and use of multiple business machines. Good visual and hearing acuity through normal corrected mechanism.
