Ariel Castro was found hanging inside his prison cell with his pants and underwear down around his ankles. Investigators notified the State Highway Patrol to consider the possibility of auto-erotic asphyxiation.

The report found that rounds by corrections officers were not done properly at the facility in Orient where Castro was serving his life prison term.
On the evening of Castro's death, the report says corrections officers missed rounds between 3 p.m. and the time Castro was found hanging from a hinge in a window of his cell by a sheet wrapped around his neck.
Investigators also found that logs were falsified during that same time period.
The state says a Bible was open to the book of John, Chapters 2 and 3.

Investigators found Castro's prison orientation was incomplete, saying he did not view the suicide prevention video. They also found that documentation regarding his medical, mental health and substance abuse screening was missing.

Castro committed suicide on Sept. 3. He had been sentenced to life plus 1,000 years for holding three women captive in his Seymour Avenue home for a decade.
The women were rescued on May 6. Castro pleaded guilty in July to avoid the death penalty.
The Ohio Department of Rehabilitation and Corrections released the following statement on the report's findings:
"Director Mohr has reviewed and accepted the recommendations made by the after action review team, and acknowledges immediate steps taken by the Correctional Reception Center to address issues of security rounds being conducted according to policy."
