11/15/2012: PROBATION MODIFIED - Doctor's request to reduce required personal appearances to every six months, reduce required random drug screens to two per month, and reduce required drug and alcohol rehabilitation meeting attendance to two per week with a minimum of ten per month granted by vote of the Board on 11/15/12.

06/05/2012: COURT ACTION - By decision and entry filed on 6/5/12, 10th District Court of Appeals affirmed the judgment of the Franklin County Court of Common Pleas, which had upheld the Board's 5/11/11 order indefinitely suspending the doctor's license and placing him on probation.

06/16/2011: COURT ACTION: By Journal Entry filed 6/16/11, the 10th District Court of Appeals dismissed doctor's appeal of the Franklin County Court of Common Pleas denial of a stay of the Board's 5/11/11 Order pending appeal.

06/08/2011: COURT ACTION: By Entry filed 6/8/11 and Decision filed 5/25/11, the Franklin County Court of Common Pleas denied doctor's motion for stay of the Board's 5/11/11 Order indefinitely suspending doctor's medical license.

06/03/2011: COURT ACTION: By Entry filed 6/3/11, the Tenth District Court of Appeals denied doctor motion for stay of the Board's 5/11/11 Order indefinitely suspending doctor's medical license.

05/16/2011: COURT ACTION: Notice of Appeal of Board's 5/11/11 Order indefinitely suspending doctor's medical license and establishing subsequent probationary conditions filed by doctor with the Franklin County Court of Common Pleas on 5/16/11.

05/11/2011: BOARD ORDER: Medical license suspended indefinitely; interim monitoring conditions and conditions for reinstatement established; interim monitoring conditions and conditions for reinstatement established; and subsequent probationary terms, conditions, and limitations established for at least five years. Based on Board's determination that doctor is impaired in ability to practice according to acceptable and prevailing standards of care based on diagnosis of cocaine abuse for which doctor has not completed required treatment at a Board-approved treatment provider. Order effective 5/12/11.

07/14/2010: CITATION: Based on doctor's alleged failure to comply with requirements applicable to physicians re-locating to Ohio after having been diagnosed or treated for chemical abuse or dependency and alleged impairment of ability to practice according to acceptable and prevailing standards of care, the underlying basis being doctor's previous treatments in New York for cocaine abuse and alcohol abuse, and on doctor's having been diagnosed, following a Board-ordered evaluation, with cocaine abuse and determined to require residential treatment and monitoring.
