12/11/2008: VOLUNTARY SURRENDER: Board Order permanently revoking doctor's medical license as authorized by doctor in lieu of formal disciplinary proceedings based on doctor's alleged inability to practice according to acceptable and prevailing standards of care by reason of mental illness or physical illness. Effective 12/10/08.

10/08/2008: PROBATION COMPLETED: Doctor's request for release from the terms of the 11/10/04 Board Order granted by Board vote on 10/8/08. Release from probation effective 10/8/08.

12/13/2006: COURT ACTION:By Entry filed 12/13/06, the Ohio Supreme Court dismissed the doctor's appeal of the 10th District Court of Appeals decision upholding the Board's 11/10/04 suspension of the doctor's medical license.

08/24/2006: COURT ACTION: Notice of Appeal of the 7/11/06 Entry of the 10th District Court of Appeals filed with the Ohio Supreme Court on 8/24/06.

07/13/2006: COURT ACTION:By Judgment Entry filed on 7/11/06 and Opinion filed 7/13/06, the 10th District Court of Appeals affirmed the judgment of the Franklin County Court of Common Pleas, which had upheld the Board's 11/10/04 suspension Order.

09/01/2005: COURT ACTION - Notice of appeal to Tenth District Court of Appeals filed by doctor on 9/1/05.

08/23/2005: COURT ACTION - By Order filed on 8/23/2005, Franklin County Court of Common Pleas granted doctor's motion for a continuation of the stay order of 12/13/2004, including the same conditions as outlined in the 12/13/2004 stay order, during the pendency of the doctor's appeal before the Tenth District Court of Appeals.

08/18/2005: COURT ACTION - By Decision filed 8/8/05 and documented by Entry filed on 8/18/05, Franklin County Court of Common Pleas affirmed Board's 11/10/04 suspension Order.

12/13/2004: COURT ACTION - By Order filed on 12/13/04, Franklin County Court of Common Pleas granted doctor's motion to stay Board's 11/10/04 suspension Order pending appeal, subject to conditions, including that doctor not 1) prescribe, purchase, order, administer or treat patients with Laetrile; 2) counsel or advise patients on the use of Laetrile; or 3) refer patients to another facility or physician for Laetrile treatment.

12/10/2004: COURT ACTION - Notice of appeal of Board's 11/10/04 suspension Order filed by doctor with Franklin County Court of Common Pleas on or about 12/10/04.

11/10/2004: BOARD ORDER - Medical license suspended for one year; subsequent probationary terms, conditions, and limitations for at least one year established. Based on doctor's plea of guilty to one misdemeanor count of Introduction or Delivery for Introduction into Interstate Commerce of an Unapproved Drug, to wit: 30 vials of Laetrile to be shipped from West Palm Beach, Florida to Salem, Ohio in violation of federal law. Order mailed 12/6/04; Order effective 12/6/04.

04/14/2004: CITATION - Based on doctor have pleaded guilty to one misdemeanor count of Introduction or Delivery for Introduction into Interstate Commerce of an Unapproved Drug, to wit: 30 vials of Laetrile to be shipped from West Palm Beach, Florida to Salem, Ohio in violation of federal law. Notice of opportunity for hearing mailed 4/15/04.
