After investigating the scene, it is believed that the Yaris was traveling southbound on Friendsville Road when it crossed the double yellow center lines to pass a vehicle ahead of it. During that attempt, the Yaris struck the Flex, which was travelling northbound, head-on.

The crash resulted in the Yaris catching fire that spread to the Flex.

The driver of the Yaris was a 16-year-old from Fredericksburg. According to the highway patrol, he was transported to an area hospital with life-threatening injuries. The 16-year-old has since died from his injuries.

The Flex had three passengers in the vehicle at the time of the crash. The driver, Amy C. Seibel, 37, of Lodi suffered serious but nonlife-threatening injuries. Seibel's children, ages 5 and 7, also suffered serious but nonlife-threatening injuries. All three were transported to a local hospital for treatment.

The incident remains under investigation, according to the Ohio State Highway Patrol.
